Nagios 2.9 NDOUtils 1.4b5 Debian 4.0ro mysql-server-5.0 problem

Friends,
 I configured Nagios 2,9 with support the NDOUtils 1.4b5 and
mysql-server-5.0, I am monitoring some hosts I believe that ndoutils this
not functioning.
it follows  nagios.log:
