Actually the Nagios server maybe on the same side of the firewall
as the exchange server.

Find the sendmail.cf file and modify this:
# "Smart" relay host (may be null)
DS

to this:
# "Smart" relay host (may be null)
DSexchange.server.fully.qualified.name

of course you change the "exchange.server.fully.qualified.name"
to be the DNS name of your exchange server.

Then killall sendmail and start it again.
