Notification Limits

If you choose to release a patch for "max_notifications", please let the list know.  For my part, I certainly think it'd be handy! :)

Set notification_interval to 0 for one notification per event.
Do some magic with escalations to get more but not zero.

In fact, I think I'll hack up a max_notifications variable for hosts and 
services. It seems overkill to use escalations for just limiting them to 
a specific number.
