nagios not sending all emails

This is getting a bit annoying and I cant figure out why nagios is doing
this.  Nagios  looks like its not sending all emails, I always seem to
receive 'OK' notifications when it never sent out a critical
notification or warning notification.   I have in the services.cfg file
the notification options w,u,c,r  .  Its weird because nagios used to
send out all notification emails correctly before and it just started to
do this.  Any ideas?
