Missing Notifications

Can someone tell me the most common reason for simply not
getting a notification for a host-down, when...

1.  contacts file says 'd,r'
2.  we got the 'r' notification...!!
3.  the host was down for app 35 min
4.  notifications are sent every 15 min
5.  we could see the host down condition as it from
	any web display
6.  history is clear and complete with a down, down, down
....not down notification was sent to the 5 contacts...but recovery
was sent...
