Host down 2 days -- only 1 notifcation

Yes -- paging continued as normal for the other hosts.

Yes.  The host was down due to a hardware issue, and was not available at 
all on the network.

The nagios log shows only one notification being sent, at the time the host 
went down.  No further notifications were sent out until I acknowledged it 
as down 2 days later.
