For what it's worth, I moderate several lists that are hosted by Yahoo, 
plus a few that are managed with Mailman software. I have those lists 
set up as follows:

1) Only members can post; non-members' messages get bounced to me. 
(This stops almost all SPAM, which mostly originates from bogus 
addresses, in its tracks.)

2) Anyone can join the list (become a member). I don't moderate that 
part, it's just a requirement to prevent SPAM.

3) Members have their first message cleared/moderated by me. (This 
seems to stop the rest of the SPAM.) Once I've cleared someone's first 
post, after that they can post at will.

4) All attachments are removed. (This inhibits the spread of viruses, 
not to mention the thoughtless practice of sending 1000 people a large 
file, when really only 1 person needed to receive it.)

I actually have to do very little moderating, per se. Very 
occasionally, I will see a bounced message from someone who I know is a 
member, but is posting from a different account. (I let those messages 
through.) Other than that. almost all of the "moderator traffic" ends 
up being SPAM, which I can cull on a daily (or so) basis at my leisure. 
For that matter, it could be set up to be automatically purged every so 
often (I don't think I'd set it for every hour, but maybe once a day) 
with very little collateral damage to the members.
