This is what you get when the user you're logged in as (with apache 
authentication) isn't mentioned in the authorized_for_system_commands.

> use_authentication=1 default_user_name=nagios

These should be on separate lines for this to work, and 'nagios' 
shouldn't have ALL permissions, since that equals NO authentication.

> authorized_for_system_information=nagios
> authorized_for_configuration_information=nagios
> authorized_for_system_commands=nagios
> authorized_for_all_services=nagios
> authorized_for_all_hosts=nagios authorized_for_all_service_commands=nagios

These must also be on separate lines.

> authorized_for_all_host_commands=nagios

I strongly suggest you add another user and DON'T give 
'default_user_name' permissions to do everything. It's a sure way to get 
annoyed by people doing the wrong thing.
