Looked to me like the template field (the first one) in your INSERT stmt was
NULL:

    David> SQL insert into `Contact` values (NULL , 'dgitman' , 'David Gitman' , 'david at domain.com' , 'dgitman at domain.com' , NULL , NULL , NULL , NULL , NULL , NULL , NULL) failed :

It should refer to something.  Try creating a contact template (even an
empty one) and referring to it from your Contact entry.
