3 e-mails per host in DOWN state

Although I haven't found something about contact_groups in your 
configuration, you may want to check if you have several users with
the same email address.  I had a similar problem when I had configured 
both a general 'nagiosadmin' user and a 'regular' user with my address:
I would get every notification twice.  If you get everything three times, 
it might be an additional notify_by_epager command (configured to send 
email) for one user?

(Sorry if that's something you already checked.)

If this doesn't help, maybe you could show us what notifications were 
logged in your Nagios event log?
