Refactor logic for next valid time

Hi list,

A couple of excellent tests by Jean Gabès - marked as todo - pointed out
problems with how nagios calculates the next valid time in a timeperiod that
has complicated combinations of exceptions and exclusions.

This patchset refactors the relevant code to fix all failing tests, as well as adds a
new one.
