I started recently with a third party product found on Nagios Exchange - (<a target="_blank" href="http://www.alarmtilt.com/nagios">http://www.alarmtilt.com/nagios</a>). It does generate phone calls using text-to-speech (sometimes funny how server names are spoken out ;-) ), sms and iphone push (I am using this mainly).
