Alerts not making it to notifications log or alert history

That's very odd, and I really don't have any concrete ideas for you. I 
haven't heard of anyone running into this issue before. Maybe 
reinstalling the packages might fix something, though?

You said it's an intermittent problem, so how often would you say it 
occurs? Have you noticed any patterns about when it happens?
