Your command check_url accepts 3 arguments:
   command_line    $USER1$/check_http -H $HOSTADDRESS$ -u $ARG1$ -w 
$ARG2$  -c $ARG3$

You provided 4 arguments:
   check_command                   check_url!foo.com!/index.jsp!5!10

"foo.com" seems to be extra. You should check it.
