What all ports need to be opened?

You don't really specify what about Nagios you are firewalling.
Inbound/outbound? NRPE?

Outbound you will need to ensure that any ports Nagios is testing are
open, NRPE uses port 5666 by default. Inbound depends on what is
trying to contact Nagios.
