Open Source Cabling Management Software?

Hey,

Can anyone make any recommendations on good cabling management software?

An IT based company here (fairly new) and we have a network rack and about
25 odd hosts and the cabling for the network rack is just getting insane.
Cables are being constantly unplugged and reconnected so, we need a database
of some kind to management what router/switch ports should plug into what
patch panel sort of thing.

Any ideas?
