Nagios Notifications incorrectly scheduled

Well, if you're asking for help figuring it out, you'll need to post  
config information and log entries. The crystal balls have all been  
snatched up by the financial industry leaving us poor computer types  
relying on real data =) There are no known issues related to  
notifications and we have to assume that nagios is doing what it's  
been told to do.

Nagios version and how installed.
OS and version it's running on.
Full host/service definitions (from status.dat preferably)
Any related escalation definitions.
nagios.log entries related to the problem host or service  
(representative snippet from start of issue)
