As to the question about the interaction between escalations and state
changes we handle that in a custom notification script that stores state and
escalation level of a particular service so it can do things like:

- reset escalation level on state change
- send recoveries to everyone who was ever notified about the current issue

I'd release it to the community but it's not even v0.01 material. A tidy up
of the code is waiting on some spare time materialising...
