mail notifications

If /var/log/maillog shows the message being sent to all three recipients I
would suggest try sending a test message from the server itself.

'echo test | mail user at domain.com'

It's possible you need to modify your notification command or your servers
smtp server to make sure the from address is proper and the forward/reverse
DNS of the server is proper.
