Implementation templates?

Hi list,

In the scope of a big Nagios implementation, I would like to know if some of you have standard documents for monitoring requirements?

Ex: how to notify, when to notify, escalation procedure, contacts etc...

The goal is to collect all data via forms (Excell, Word docs, ...) then implement the solution.

Any advice?
