we have set up exactly what you refer to in our Data center , we have
the  nagios send to the asterisk a request for a call with the number to
call  and the asterisk is calling the person's number either cell phone
or land line number ( depends on what is defined in the nagios ) .

True that our "calling" service doesn't tell you what the problem is -
for that we still need to reach a computer , but the main issue is that
we are awake ( or mostly awake) and actually aware that there is a
problem .

in order for this to work you'll have to have asterisk running (Duh) and
able to dial out .
then you need to configure an extension ( * see asterisk documentation )
and allow access to the asterisk form the nagios (in the manager.conf
file *) .

As for the different messages - that is something you can try and play
with - either with festival like other recommended or via sending
parameters to the asterisk .
