RRD tool allows you to graph constants (gauge) or rates (counter/derive).
Various Nagios plugins/add-ons allow interaction with RRDtool.

A hard drive filling up at a certain rate is akin to a traffic counter
ticking up on a router.
You'll want a plugin or polling method that returns the total disk
utilization in bytes or MB.

Im not familiar with the data format used in snmp-based disk checks, but it
should be fairly simple to insert the data into an RRD file.  Creating the
RRD as a counter or derive type should fit the bill.
