I think the Nagios way is to create two contacts and have one of them
point to your email address and have notification options to
alert on warn / crit, and a second contact that only alerts on
crit that points to your pager.

Alternatively, pass both the email and pager address to your
notification command and have it decide based on severity how to
route the alert.
