What troubleshooting have you done so far?

Is SNMPd running on the remote host? It seems like it's not. Can you
query the OID with snmpget from your nagios host? If not do you have a
firewall enabled that's restricting access? Are you allowing access via
snmpd.conf? Are there routing problems between your nagios host and the
remote host? Can you run the check_command, exactly as it's specified in
your configs, as the nagios user?

For future reference, you'll likely get more informative and useful
responses if you include pertinent information such as program versions,
OS's, specific error messages, specific relevant configuration snippets
and any troubleshooting and the results that you've already done.
