Nagios Plugins Performance Data

I'm using version 1.4 of the Nagios plugins.  I trying to find information 
on the format of the performance data for each of the plugins so I can 
update my configuration for rrdtool (using Nagiosgraph).

eg.
# ./check_disk -w5% -c3% /
DISK OK - free space: / 55162 MB (83%);| /=11045MB;62896;64220;0;66207

What is the format used by check_disk and the others?  I have looked for 
it for half a day and couldn't find it.  I'm sure its somewhere easy to 
find.
