Mmmmm. AS 4.0. Do you have SELINUX enabled? Do you see SELINUX errors in
/var/log/messages? Make sure your policy allows apache to write to that
file/directory.

Another common problem is that apache is not a member of the nagiocmd
group _or_ wasn't restarted after adding it to that group. Your apache
error logs might be informative in that regard as well.
