The plugin is missing, is unreadable due to permissions or the path to
it in (mis)commands.cfg is incorrect.

The plugin, when the attempt to execute it was made, didn't return any
output.

You're missing the libexec directory so they're not installed here it
seems. Did you download and install them? They come separate from the
main nagios distribution: http://www.nagios.org/download/ or
http://nagiosplug.sf.net. If did download and install them, make sure
that you passed the same non-standard path information to the configure
script for the plugins that you did for nagios.
