nagmin mysql error

I have a new install (nagios 1.1, nagmin 2.0.6 and mysql 3.23.53)
and I'm having problems adding a host through webmin. After inserting
the data and clicking on "add" I get the following:

  SQL insert into `Host` values (NULL , 'csproddb' , NULL , NULL , NULL , ' '
  , NULL , NULL , NULL , NULL , NULL , NULL , NULL , NULL , NULL , NULL) failed :


I'm not seeing a log of this insert attempt anywhere, so I have no idea
what the problem is. This isn't limited to the Host table, I get the same failure
on any table. Anyone else run into something similar or have any ideas? TIA
