NagMIN Table Edit Failure

When I try to add a contact via NAGmin I get
SQL insert into `Contact` values (NULL , 'dgitman' , 'David Gitman' , 'david at domain.com' , 'dgitman at domain.com' , NULL , NULL , NULL , NULL , NULL , NULL , NULL) failed :
Is it not reaching the database?
