You say you've run this from the command line, but have you run it as the
Nagios user?  check_by_ssh requires that you have a passwordless ssh key to
allow login to the remote host... configuration of that key is user-dependant.

It's possible you're getting an UNKNOWN result from check_by_ssh because it is
unable to login to the remote host.
