The easiest way is to open up Control Panel -> Administrative Tools
-> Performance, click the + above the graph, and all the
objects/counters/instances are there for you to examine. You should
be able to figure out the format from the examples you gave.
Hint: open up the "Explain" window, it helps lots. :)

is not as useful as it could be, but it's there if you need it.
