Please post the pre-flight check output to the list, and maybe attach your
checkcommands.cfg file as well.

This should help the list determine what the problem is.

Unfortunately backing down Apache isn't really an option - but I don't
think that's the problem...

I'm getting errors in the checkcommands.cfg file - it doesn't like a
single command.  If I run them live in lib_exec it works fine.  I wonder
if the HOSTADDRESS and ARGS1 variables aren't getting set - I couldn't
find them in any config file - where are those?
