Cisco Switch Port States Plugin

I want to monitor cisco switch port states like the following
   
  Port 1     Active/forwarding
  Port 2     Down/Disconnenct
  Port 3     Active/Block 
   
  Is there any plugin or How to proceed with this? Any one is having some script for this please post here.
