action_url and notes_url as macros

We point at status.cgi?host=$HOSTNAME$. It's a better overview if you 
ask me.

I'm referring to the hostextinfo variables notes_url and action_url. 
action_url is only available in 2.0. It would be really neat to have 
them sent in an email so admins can respond in the proper manner.
