Example

The topic of how to get warnings via email and critical alerts via pages
came up on nagios-users the other day.  After a bit of back-and-forth I
think the conclusion was that the documentation covers the topic but was
lacking in examples.  Seeing none in the notifications section already, I
added an Examples section and that particular example.  Attached is a
context diff.
